Abstract: Observations of B[e] supergiants suggest the presence of a circumstellar disk (or at least an equatorial density enhancement). However, little is known about the structure of these disks or how they form. In this talk, I will review various models of disk formation, ranging from bi-stability to viscous decretion, which predict quite different geometrical and kinematic structures for the disk. Finally, I will present the results of our 3-D NLTE Monte Carlo radiative equilibrium simulations, which predict the spectral energy distribution, intrinsic polarization, and spectral line profiles required for testing the models of B[e] disks.
